OCR description
Parkland Community Health Plan, the covered entity (CE), reported that an employee of a vendor used by its business associate (BA) mailed the protected health information of 1,682 individuals to the wrong recipients. The PHI involved included names, addresses, dates of birth, and medical record numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE and BA implemented additional administrative and technical safeguards.
