OCR description
The business associate (BA), Horizon Actuarial Services, LLC, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 227,953 individuals. The PHI involved included names, dates of birth, Social Security numbers, claims and financial information, and diagnoses. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative and technical safeguards.
