OCR description
Gainwell Technologies, the business associate (BA), reported that it experienced a security incident when an employee provided an unauthorized user with credentials to access its computer systems. This breach affected the protected health information (PHI) of 1,955 individuals. The PHI involved included names, member identification numbers, and diagnoses. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
