OCR description
The covered entity (CE), Highmark, reported that a subcontractor of its business associate (BA) experienced a cybersecurity incident that affected the protected health information (PHI) of 67,147 individuals. The PHI involved included names, dates of birth, medication information, and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect its PHI.
