OCR description
The covered entity (CE), Local 295 IBT Employer Group Welfare Fund, reported that its business associate (BA) experienced a cybersecurity attack that affected the protected health information (PHI) of 6,123 individuals. The PHI involved included names, addresses, dates of birth, and Social Security numbers. The CE notified HHS; the BA notified affected individuals, the media, and law enforcement. In its mitigation efforts, the BA implemented additional administrative safeguards and retrained its employees in an effort to better protect PHI. OCR provided technical assistance to the CE regarding the HIPAA Privacy Rule.
