OCR description
The covered entity (CE), Virginia Mason Medical Center, reported that multiple employees were the victims of an email phishing attack affecting the protected health information (PHI) of approximately 2,733 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, lab results, medications prescribed, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative, technical, and security safeguards and retrained its staff on email security.
