Livingston Memorial VNA Health Corporation and its affiliates Livingston Memorial Visiting Nurse Association and Livingston CareGivers, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of approximately 34,775 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, claims and financial information, diagnoses, lab results, medications and other treatment information. The CE notified the impacted individuals, HHS, and the media. In its mitigation efforts, the CE revised and implemented additional HIPAA policies, enhanced its cybersecurity controls, and applied appropriate safeguards to protect against unauthorized access to its PHI.
