OCR description
The covered entity (CE), Resources for Human Development, reported that a computer drive was stolen which compromised the protected health information (PHI) of 46,673 individuals. The PHI involved included diagnoses, conditions, medications, Social Security numbers, and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional physical and technical safeguards and provided complimentary credit monitoring services to affected individuals.
