OCR description
The State of Arizona Department of Administration, the covered entity (CE), reported that its business associate (BA) mailed the protected health information (PHI) of 820 individuals to the wrong recipients. The PHI involved included names, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards.
