OCR description
The covered entity (CE), Bishop Eye Center, reported that it was the victim of a cyber-attack affecting the protected health information (PHI) of 2,292 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services, implemented additional technical safeguards, and revised its policies and procedures.
