OCR description
The covered entity (CE), Georgia Pines CSB, reported that its facility was burglarized and the protected health information (PHI) of 24,000 individuals was compromised. The PHI involved included names, addresses, Social Security numbers, and medical records. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE increased its physical security safeguards and provided HIPAA training to its employees. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
