OCR description
The covered entity (CE), Chinese Community Health Plan, reported that due to a coding error, the protected health information (PHI) of 502 individuals was sent to the wrong recipients. The PHI involved included names, addresses, and claims and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE corrected the coding error, revised its policies and procedures, and retrained its employees.
