OCR description
The covered entity (CE), Healthcare Collections-I, reported that an employee of its business associate mailed the protected health information (PHI) of 900 individuals to the wrong recipients. The PHI involved included names, addresses, and other clinical and treatment information. The CE notified HHS and affected individuals. In response to the breach, the CE corrected the mailing error and implemented additional technical safeguards.
