OCR description
The covered entity (CE), HealthActions, reported that employees were the victims of an email phishing scheme that compromised the protected health information (PHI) of 3,653 individuals. The PHI involved included names, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members on email security.
