OCR description
The covered entity (CE), Fairfield County Implants and Periodontics reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of 10,502 individuals. The PHI involved included names, Social Security numbers, birthdates, medications, diagnoses, and claims and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach strengthened its administrative, technical, and security safeguards. Employees were retrained on email security.
