OCR description
The covered entity (CE), The Mental Health Center of Greater Manchester, reported that its business associate (BA) experienced a ransomware incident that affected the protected health information (PHI) of 1,322 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring services.
