OCR description
The covered entity (CE), Montefiore Medical Center, reported that an employee impermissibly accessed the protected health information (PHI) of 3,717 individuals. The PHI involved included names, dates of birth, addresses, email addresses, phone numbers, health insurance information, and partial Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards to better protect its sensitive data.
