Select Benefits Group, LLC dba Dental Select

The business associate (BA), Select Benefits Group dba Dental Select, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 1,035 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative and technical safeguards. OCR provided technical assistance to the BA regarding its HIPAA Security Rule obligations.
