OCR description
The covered entity (CE), Allied Eye Physicians and Surgeons, Inc., reported that its business associate (BA) was the victim of a hacking incident that affected the protected health information (PHI) of 20,651 individuals. The PHI involved included names, addresses, drivers’ license and Social Security numbers, diagnoses/conditions, medications and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. To mitigate harm, the CE implemented additional technical safeguards.
