Sterling Healthcare Holdings, LLC DBA Cordant Health Solutions

The covered entity (CE), Sterling Healthcare Holdings dba Cordant Health Solutions, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 20,932 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, claims information, lab results, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to improve its security posture. OCR provided technical assistance to the CE regarding the HIPAA Security and Breach Notification Rules.
