OCR description
The covered entity (CE), Frank Eye Center, reported that its business associate (BA) was the victim of a cyber-attack that compromised the protected health information (PHI) of 26,333 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, diagnoses, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and ceased its business relationship with the BA.
