OCR description
The covered entity (CE), Guardian Pharmacy of Cincinnati dba Mullaney’s, A Guardian Pharmacy, reported that an employee emailed the protected health information (PHI) of 1,720 individuals to the wrong recipient. The PHI involved included names, addresses, and medications. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. The CE’s mitigation efforts included administrative, technical, and security safeguards to better protect its sensitive data. All staff were retrained.
