OCR description
Cypress Healthcare, the covered entity (CE), reported that a laptop was stolen from an employee’s car. This breach affected the protected health information (PHI) of 899 individuals. The PHI involved included names and dates of birth. The CE notified HHS, affected individuals, the media, local law enforcement, and posted substitute notice to its website. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect PHI. OCR provided technical assistance to the CE regarding the HIPAA Breach Notification Rule.
