OCR description
The covered entity (CE), EmblemHealth Plan, reported that an employee inadvertently mailed the protected health information (PHI) of 11,399 individuals to the wrong recipients. The PHI involved included names, member identification numbers, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative and technical safeguards to better protect its PHI.
