OCR description
The business associate (BA), Health Care Administrative Services, reported that one of its employees removed the protected health information (PHI) of 500 individuals from its computer systems. The PHI involved included names, Social Security and drivers’ license numbers, addresses, dates of birth, diagnoses, medications prescribed, and financial information. The BA notified HHS and affected individuals. In response to the breach, the BA strengthened its administrative and technical safeguards to better protect its sensitive data.
