The covered entity (CE), Washington University School of Medicine, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 500 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, medications, and health insurance information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
