Stephen Sims, MD, PA, dba North Lakes Pain Consultants, the covered entity (CE), reported that it was the victim of a ransomware attack that compromised the protected health information (PHI) of 8,620 individuals. The PHI involved included names, Social Security numbers, dates of birth, phone numbers, email addresses, diagnoses, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained it staff.
