OCR description
The covered entity (CE), Virginia Commonwealth University Health System, reported that a technical issue with a new electronic medical record system and patient portal platform caused the protected health information (PHI) of 4,441 patients to be misdirected to the wrong recipients. The PHI involved included names, Social Security numbers, birthdates, lab results, medical record numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect its PHI.
