OCR description
The covered entity (CE), Platinum Hospitalists, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 5,535 individuals. The PHI involved included names, dates of birth, diagnoses/conditions, and claims information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its workforce members and implemented additional technical safeguards.
