OCR description
The covered entity (CE), Shoreline Eye Group, reported that its business associate (BA) was the victim of a breach affecting the protected health information (PHI) of 57,047 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, and diagnoses and conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional technical safeguards to better protect PHI.
