OCR description
Virginia Mason Medical Center, the covered entity(CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,523 individuals. The PHI involved included names, addresses, birthdates, Social Security numbers, email addresses, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach incident, the CE implemented additional administrative, technical, and security safeguards.
