OCR description
The covered entity (CE), Smile Solutions, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 8,000 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and provided its employees with security awareness training.
