OCR description
The covered entity (CE), Heartland Healthcare Services, reported that it was the victim of a cyber-attack affecting the protected health information (PHI) of 2,763 individuals. The PHI involved included names, addresses, dates of birth, medications, and Social Security numbers. During OCR’s investigation, the CE closed its business. As a result, OCR no longer has jurisdiction over this matter.
