OCR description
The covered entity (CE), Precision Eye Center, reported that its business associate (BA) experienced a cyber-attack that affected the protected health information (PHI) of approximately 58,462 individuals. The PHI involved included names, addresses, Social Security numbers, birthdates, and diagnoses. In response to the breach, the CE notified HHS, affected individuals, the media, and provided substitute notice. Subsequently, the CE terminated its business relationship with the BA.
