OCR description
The covered entity (CE), Carolina Eyecare Physicians, reported that its business associate (BA) experienced a breach affecting the protected health information (PHI) of 68,739 individuals. The PHI involved included names, address, dates of birth, lab results, diagnoses, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE terminated its business relationship with the BA, implemented additional technical safeguards, and provided complimentary credit monitoring services.
