New Schryver, LLC d/b/a/ TridentCare

The covered entity (CE), New Schryver, LLC dba TridentCare, reported that it experienced a burglary at one of its facilities and hard drives and other equipment containing the protected health information (PHI) of 6,190 individuals, were stolen. The PHI involved included names, Social Security numbers, and dates of birth. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring and identity theft services and enhanced its administrative, technical, and physical security safeguards at its locations. The CE also centralized satellite servers to secure its data centers.
