Sharper Vision P.A.

The covered entity (CE), Sharper Vision, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 6,891 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In its mitigation efforts, the BA implemented additional administrative and technical safeguards to better protect PHI.
