OCR description
The covered entity (CE), ARC Industries of Franklin County, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 22,011 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, claims information, and diagnoses and conditions. The CE notified HHS, affected individuals, the media, and posted substitute notice. The CE implemented additional administrative and technical safeguards.
