OCR description
The covered entity (CE), Alabama Eye & Cataract, reported that its business associate (BA) experienced a hacking attack that affected the protected health information (PHI) of 25,900 individuals. The PHI involved included names, addresses, Social Security numbers, telephone numbers, and diagnoses and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE severed its business relationship with the BA.
