OCR description
The covered entity (CE), Providence St. Joseph Health, reported that its business associate (BA) experienced a cyber-attack affecting the protected health information (PHI) of 7,229 individuals. The PHI involved included names, gender, dates of birth, Social Security numbers, addresses, telephone numbers, email addresses, and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the BA implemented additional administrative and technical safeguards and provided complimentary credit monitoring and identity theft protection services.
