OCR description
The covered entity (CE), Kevin Wolf, DPM dba Goldsboro Podiatry, reported that its business associate (BA) experienced a ransomware attack that affected the protected health information (PHI) of 30,668 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional technical safeguards and the CE provided complimentary credit monitoring services to affected individuals.
