OCR description
The covered entity (CE), Catholic Health System, reported that its business associate (BA) was the victim of an email phishing attack affecting the protected health information (PHI) of 904 individuals. The PHI involved included names, dates of birth, medical record numbers, and other treatment information. The BA notified affected individuals; the CE notified HHS, provided media notice, and posted substitute notice on its website. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
