Community of Hope D.C.

The covered entity (CE), Community of Hope D.C., reported that an employee was the victim of an email phishing attack that compromised the protected health information (PHI) of approximately 580 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided affected individuals with complimentary credit monitoring services, implemented additional technical safeguards, and trained employees on cybersecurity.
