OCR description
The covered entity (CE), Southwest Health Center, reported that it experienced a hacking attack that affected the protected health information (PHI) of 46,142 individuals. The PHI involved included names, addresses, dates of birth, social security and drivers’ license numbers, diagnoses, lab results, financial information, and medications. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
