OCR description
The covered entity (CE), Kurilec Eye Care, LLC, reported that its business associate (BA) was the victim of a breach that compromised the protected health information (PHI) of 2,876 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, and clinical diagnoses and conditions. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website.
