OCR description
The covered entity (CE), McLaren Port Huron, reported that its former business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 48,957 individuals. The PHI involved included names, addresses, dates of birth, email addresses, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website.
