Blue Shield of California

The covered entity (CE), Blue Shield California, reported that a subcontractor of its business associate (BA) was the victim of a ransomware attack affecting the protected health information (PHI) of approximately 1,506 individuals. The PHI involved included names, subscriber identification numbers, diagnoses, medications, addresses, dates of birth, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring and identity theft protection services and implemented additional technical safeguards.
