Advantage Benefit Solutions, a division of Patriot Growth Insurance Services, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 4,614 individuals. The PHI involved included names, addresses, birthdates, Social Security numbers, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical and security safeguards. Staff were retrained on email security.
