OCR description
Center for Primary Care, the covered entity (CE), reported that it experienced a cyber-attack that affected the protected health information (PHI) of 2,400 individuals. The PHI included names, claims information, health insurance information, addresses, and medication information. The CE notified HHS, affected individuals, and the media. The CE strengthened its administrative, technical, and security safeguards to better protect PHI.
