OCR description
The business associate (BA), OneTouchPoint, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 4,112,892 individuals. The PHI involved included names, addresses, dates of birth, diagnoses/conditions, medications, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. OCR provided technical assistance to the BA.
